We understand your time is valuable. We make every effort to remain on schedule. If you are 15 minutes late, your appointment may be cancelled in fairness to the patients who have arrived for their appointments on time. It is also very important to be clear what your needs are when you schedule your appointment so we may allow enough time for your appointment. When scheduling your appointment, please leave us a phone number where you think you will be reachable on the day of your appointment.

Due to the nature of obstetric practices, our physicians may be called out suddenly for deliveries and emergencies. We will make every effort to notify you as soon as possible if your appointment will be delayed or cancelled. We appreciate your patience and understanding.

Please notify us at least 24 hours in advance if you are not able to keep your appointment. This enables us to contact someone who may need that appointment time. Failure to do so will result in a rescheduling charge.

Please make sure to bring the following information:

  • Drivers License
  • Insurance Card
  • List of current medications-prescription, over the counter medications and supplements
  • Method of payment
  • Medical records pertinent to your care – ultrasound reports, lab results, records from another physician

Our telephone lines are open throughout the day. Please feel free to call anytime for questions and concerns.

Non-urgent and routine messages are typically returned by the staff according to medical priority. The nurses line is checked hourly and urgent phone messages are answered throughout the day. General messages left after 4 pm will be answered the following day.

Please contact your pharmacy initially for refill requests. If you have been directed to contact our office, please be sure to have the pharmacy name, phone number, and the name of the medicine for refill. Requests for prescription refills are only done during office hours and may take up to 48 hours to complete. Please be sure to call at least 4 business days before you run out of your medication.

After hours, please leave a detailed message to include your name, date of birth, and telephone number and we will return your call the next business day. If you have a medical emergency, please proceed to the nearest emergency department and hospital for immediate care.

Our staff members are held up to the highest confidentiality standards. Your medical information will remain completely confidential unless we receive a signed written release from you. NO INFORMATION will be shared with your spouse or family members unless specific authorization is obtained to do so.

We can forward your medical records to other physicians and medical facilities free of charge for continuity of care. Records copied for patients, insurance companies, or for any other reasons will be charged a fee.

Payments for co-pays and fees for services rendered are due at the time of service. We accept cash, check, debit cards,Visa, Mastercard, American Express, and Discover cards.

You are responsible for knowing your insurance limitations and policies, as well as understanding what your financial obligation is (deductible, co-insurance, co-pay’s, etc.) Please contact your insurance company before your appointment to be assured that you have coverage for the services you will be receiving.

If your insurance requires a referral or authorization for your visit and you do NOT have this with you at the time of service, your appointment may need to be rescheduled.

Please bring a current copy of your insurance card to EACH visit. We cannot submit a claim to your insurnace company without specific information listed on the card.

We are happy to submit your bill to the insurance company; however, ultimately the payment is your responsibility.

Insurance Information

You must present a valid insurance card with EACH visit to our office. Inaccurate billing information, expired insurance cards, or failure to obtain proper authorization will result in payments being your direct responsibility.

It is your responsibility to follow the rules and regulations outlined by your specific insurance policy. You are responsible for understanding what your financial responsibility would be (deductibles, co-insurance, and copays) as well as what your coverage limitations are (covered and non-covered benefits, required referrals, pre-authorizations, etc). You can find this information on your insurance company’s web site or obtain the information from your employer.

Cancellation Policy

Patients scheduled for cosmetic procedures must cancel their appointments at least 48 hrs prior to their scheduled appointment time via TELEPHONE CALL – or they will incur a $50 charge. Medical appointment no shows will incur a $25 charge if their appointment is not cancelled at least 24 hrs prior to their scheduled appointment time.
